offerts.<\/li>\n<li>Cliquez sur\u00a0<strong>\u00ab Choisir \u00bb<\/strong>\u00a0via\u00a0<strong>\u00ab Thermopompe efficace \u00bb<\/strong>.<\/li>\n<li>Appuyez sur\u00a0<strong>\u00ab Continuer \u00bb<\/strong>\u00a0pour acc\u00e9der au reste du formulaire.<\/li>\n<li>Remplissez ensuite chacune des prochaines informations demand\u00e9es pour conna\u00eetre le\u00a0<strong>montant exact de l\u2019aide financi\u00e8re<\/strong>\u00a0auquel vous \u00eates admissible\u00a0:<\/li>\n<\/ol>\n<ul>\n<li><strong>Indiquez la date d\u2019installation<\/strong> de votre thermopompe (format JJ\/MM\/AAAA), telle qu\u2019indiqu\u00e9e sur votre facture.<\/li>\n<li><strong>Entrez le num\u00e9ro AHRI<\/strong> de votre mod\u00e8le de thermopompe (visible sur votre facture).<\/li>\n<li><strong>Ajoutez l\u2019adresse d\u2019installation<\/strong> en cliquant sur \u00ab\u202fAjouter une adresse\u202f\u00bb, puis s\u00e9lectionnez-la une fois inscrite.<\/li>\n<li><strong>Joignez les documents requis \u00e0 votre demande (<u>Photos<\/u> de vos pi\u00e8ces justificatrices)<\/strong>\n<ul>\n<li>Fournir une copie \u00e9lectronique de l\u2019un des deux documents suivants comme preuve de propri\u00e9t\u00e9 :\n<ul>\n<li>Relev\u00e9 de taxe fonci\u00e8re (compte de taxes municipales)<\/li>\n<li>Premi\u00e8re page de l\u2019acte de vente (titre de propri\u00e9t\u00e9)<br \/>\n<em>Le pr\u00e9nom et le nom de la personne faisant la demande doivent correspondre \u00e0 ceux indiqu\u00e9s dans le profil LogisVert, sur la preuve de propri\u00e9t\u00e9 et sur les factures.<\/em><\/li>\n<\/ul>\n<\/li>\n<li>Joindre une copie \u00e9lectronique de la facture d\u00e9taill\u00e9e attestant de l\u2019achat et des travaux d\u2019installation de la thermopompe admissible.<\/li>\n<li>Fournir une photo claire de la plaque signal\u00e9tique de l\u2019unit\u00e9 sur laquelle figurent le num\u00e9ro de mod\u00e8le et le
